This form is for those that wish to offer a program through the Essex Junction Recreation and Parks Department. Please take your time and thoughtfully answer the following questions about the program you would like to teach. You should be directed to a confirmation page and receive a confirmation email if your application has been successfully submitted. If you are not redirected when you click "Submit" please check over your answers to be sure you have completed all required fields. Your application will be reviewed by the Assistant Director of Programs. You will be contacted within two weeks of submitting your application.
